Regarding affiliations, for the "Foldit Players" consortium, we typically don't add any additional affiliation. The assumption is that your contributions as a game player are done as a private individual. (That is, even if you have an affiliation to an academic or scientific organization, you're not playing the puzzle as part of your duties to that organization.)
